Lifestyle Medicine of Lake Oconee offers integrated services to support your overall health and well-being using evidence-based therapies identified by Medicare and the National Institutes of Health.

Annual Wellness Visit

The Annual Wellness Visit is funded by Medicare to improve the health of seniors, help to prevent the onset of disease or to slow the progression of existing conditions.

A Personal Prevention Plan is provided for patients, reviewed and explained by a clinician, with an offer to schedule any of 28 appropriately identified screenings.  There is no co-pay for Medicare

Exercise

Research has shown that doing just 30 minutes of exercise, five times a week can prevent or manage heart disease and diabetes, keep bones and joints strong and reduce feelings of stress,  anxiety and  depression.  Referrals are made to Silver Sneakers, Health Clubs and Physical Therapists.

Wellness Counseling

According to the NIH, older adults with three or more chronic health conditions have a 30 – 60% chance of also experiencing depression or anxiety.   Therapy offered to patients and their families to address symptoms of stress, anxiety, depression, caregiver distress and grief that often accompany chronic or serious illness. Reimbursed by Medicare and most major insurances.

Online Wellness Programs

For Stress, Nutrition and Sleep are available to patients with care coordinated by Lifestyle Medicine of Lake Oconee and the ClevelandHeartLab®.

Metabolic Weight Management

Medically supervised and multidisciplinary weight management program for patients with specific risk factors for chronic disease and obesity.  Group therapy offered in parallel to identify and address emotional factors associated with weight issues.

Memory Screening

Establishes a baseline of neurocognitive functioning to detect early or slight cognitive impairments.  The screening measures verbal memory, psychomotor speed, processing speed, simple visual attention and motor speed.  Follow up testing provides a longitudinal report over time to track disease progression, outcomes or treatment effects.  Referrals made to an internal medicine physician or neurologist if impairment is identified in the screening.  Counseling available to patients and their families for life transition strategies.

Health Seminars

FREE health seminars and programs in coordination with the Rehabilitation and Performance Center, Georgia Agency on Aging and the North Georgia Heart Foundation to promote healthy lifestyles for our community.

Lifestyle Medicine of Lake Oconee, a health initiative sponsored by Cardiology Care Clinics, is a center of highly skilled and certified professionals using evidence-based interventions and lifestyle coaching to support you in making healthy choices to improve your quality of life.

Heather Chronos, PA-C

Certified Physician’s Assistant

Heather Chronos is a Certified Physician’s Assistant with Cardiology Care Clinic of Lake Oconee. She helped design the clinic and is responsible for the operations of the clinic.

Prior to working with Cardiology Care Clinic, Heather supervised the congestive heart failure management program at Heart Alert, one of the region’s first community-based disease state management companies. She also worked for six years at Emory University Hospital where she managed care for angioplasty patients and critical care unit patients.

Linda Calvo Kimbell, LCSW

Clinical Social Worker in Gerontology

Chronic disease is often accompanied by difficult emotions – stress, anxiety, depression and sometimes caregiver burnout. Linda is specially trained to support patients, and their families, to learn new skills and insight that will allow them to enjoy full and meaningful lives. She believes that, with compassionate and confidential counseling, the impact of grief and loss can present opportunities for new beginnings. Linda is trained in Cognitive Behavioral Therapy, Supportive Therapy and Palliative Care. She works with our other clinicians in an integrated team approach to support the whole person – body, mind, spirit and relationships. Counseling is offered through individual and/or group therapy.

Leigh Ann Stephens, MSN, NP-C

Nurse Practitioner

Leigh Ann Stephens, MSN, NP-C is a Board Certified Adult and Gerontology Nurse Practitioner. She received her bachelor’s degree in nursing at Georgia College & State University. As a Registered Nurse, she worked in various intensive care settings with the majority of experience being immediate post-operative cardiology care. She then went on to attend University of Alabama at Birmingham for her master’s degree. She is a member of American Association of Nurse Practitioners.

At Cardiology Care Clinics, Leigh Ann provides cardiac evaluation and care as well as complete primary care services. She has initiated a cardio-metabolic clinic which is a medically supervised and multidisciplinary weight management program. Being a Lake Oconee native, she has very close ties to the community and feels fortunate to be able to serve the people of her hometown for a variety of their medical needs.

Eric Silver, MD

Eric Silver, MD

Certified Obstetrician & Gynecologist

Dr. Eric Silver has been practicing Women’s Health in the Lake Oconee area for the past seven years. Originally from London England, he completed medical school at Charing Cross Hospital. He then pursued his education in the USA where he completed 2 years of a surgery residency before completing his residency in Obstetrics and Gynecology at Washington University (Barnes Hospital) in St. Louis. He is board certified in Obstetrics and Gynecology and is a fellow of the American College of Obstetrics and Gynecology.

Dr. Silver practiced in Cincinnati from 1979 to 2010, where he was directly involved in opening the Labor and Delivery unit at Mercy Hospital Fairfield. There he served as the director of the department of Ob-Gyn for 10 years.

In 2010 he moved to the Lake area to continue practicing Gynecology with St Joseph’s medical group. When St Mary’s acquired the hospital, he was invited to join their medical group. As of March 1, Dr. Silver is excited to pursue the opportunity to deliver integrated care at Life Style Medicine of Lake Oconee, a practice of “like-minded clinicians” who collaborate with Cardiology Care Clinics.

Dr. Silver offers a comprehensive approach to Women’s Health Care, and feels passionately that the role of the gynecologist is not only assessing problems of the female reproductive tract, but also be involved in a comprehensive approach to the patient in making sure overall health and screenings are appropriately addressed. Dr. Silver brings extensive experience in the management of breast health, heavy periods and fibroids, female cancer, all aspects of menopause and sexual health, incontinence issues, bone health and much more.

In today’s world, it is hard to navigate through the process of referral to specialists. He strongly believes in his role as a patient advocate and is always involved in direct communication with specialists and primary care providers. Dr. Silver will help to facilitate appropriate referrals and appointments. He makes himself available to patients and believes in the value of direct communication, and is happy to discuss any concerns of care that arise.

In his spare time he loves to travel, play golf and spend time with his wife, children and seven grandchildren.
